It has been confirmed that Countess Alexandra has recently had a facelift:

It was noticed during Felix' graduation that Alexandra had what looked like fresh scars in her face after what appeared to be a face lift.
Her secretary/PR-associate Helle von Wildenrath Løvgreen says to the tabloid BT that: "The Countess feel that the exterior should match the interior. There are many modern women who gets facelifts and recently the Countess got, what you call a mini-facelift.
BB has more:

Helle von Wildenrath Løvgreen said to BB that:
"Yes, I can confirm that. Countess Alexandra freely admits/acknowledge that she has had a surgical procedure.
Not only because it's obvious but also because the Countess feels that modern women should stand by their choices and not be embarrassed over the fact that they get professional help. It has over time become a natural thing to have done and it's a minority who can say that they haven't had something done.
That is something Countess Alexandra freely admits she has. Eve though it's a private matter Alexandra thinks you should be open and honest for example having cosmetic surgery and do it, if you yourself think something needs to be changed."
Only a week prior to the graduation Alexandra had her eyes fixed and had a small facelift that makes the lower face and neck look better.
Q: But why? She's beautiful as is.
"Yes, but I can only say that Countess Alexandra has felt that her exterior should match her interior. It's after all a personal feeling and completely natural wish."
- Well, while cosmetic surgery has certainly become much more common in DK, I'm far from certain that it is something the majority undergo.
In the segment Alexandra may mingle, but not among the general public as a whole.
Apart from that I really don't care. Her money, her face.
I don't think she needed a facelift though. She looked perfectly well for a women her age IMO.
But if she's happy, I'm happy.
